The lore has always been Isaac dies because of the abuse he suffers from. The new ending just makes Isaac the loremaster.

Probably the lighting or angle - but Maggie looks a bit older

According to the new ending by defeating the beast, the Isaac that dies in the game is implied to not be the real Isaac. The real Isaac could be illustrating the stories for his dad to tell him, and you play those adventures.

Maggie is a princess, and deserves to be treated like one.

I would never have recognised Isaac without the description. I don't think I've ever seen him without the tears.
