The dilemma, you can get an old doctor experienced in pulling your teeth without pain and willing to give you full anesthetic or you can get her, a hot demoness to pull your teeth but in a lot of pain, probably zero anesthetic and she enjoy it to the point of orgasm.

Finally one of the excruciatingly rare DOTA comics that I didn't need knowledge on the game to be funny. The punchline is something virtually everyone can understand and grasp and not require archaic knowledge of not only a game but it's meta. Please keep doing future DOTA comics like this.
