Replace the Sandvich with an MP7M Healing Dart, the Heavy with a Field Medic and the Mini-Medic with an injured Teammate and you get the Killing Floor version of this strip.

Oh, I dunno. On my regular server, most regulars use mics, and if a heavy notices a medic's hurtin' (and sometimes the medic will ask for the sandvich), he'll usually ask him to stand still and then toss it right at him.
Sandvich on ground = a non-medic often picks it up by accident.

The only times I have seem a Medic calm down to take a Sandvich or ask for a Sandvich are those who use voice commication wisely.
In fact, most Medics who use voice commication wisely pretty much wipe the team when they buddy up with a smart player.

A medic is like a deer on a freeway. We're so busy trying to heal people and dodge that we honestly don't see you toss a sandvich, and consider it suspect that someone threw something at us that isn't armed.

In defense of medics, we usually don't expect heavies to bother trying to heal us, no more than we would expect them even to notice if we were taking damage. We're too concerned with being on fire and trying to preserve our remaining 30 hitpoints, instead of waiting for a miracle.

@Denilson Sá: Yeah, if you made 3 buckets in a row with none for your opponent, you lit on fire and it improved your shot accuracy. It also prevented you from being called for goaltending.
Also NBA Jam had Charles Barkley. Barkley was the shit!

@Denilson Sá: I believe it was player. There was, however, something called "team fire" which affected your whole team and lasted like 20 seconds instead of lasting until the other team finally scored a shot. Don't remember how you could get team fire though.

Silly Mini-Medic, you must ALWAYS bring a portable friendly Pyro with you in case that happens!
For people who never played NBA Jam, if a player (or a team, I don't remember) scores several shots without other player (or team) scoring, the narrator says "He's on fire!", and also the ball becomes a fire ball (including putting fire at the basket). Oh, playing NBA Jam was a lot of fun!

@Mr. Wise Guy: Maybe they should make a small arrow? Like the one when you enable the option to have an arrow above the heal target, but it would go around the screen too like the medic call.

That aside, I would have to say that the medic is usually on fire because the rest of the team steals the medkit you so rightfully deserve; and only rarely do heavies give you their sandvich.
