On the "Guro" thing, it's also not necessarily mean violence/death. It can apply to anything grotesque, such as scat, bugs and other such things. Or what Frid said. Sexual violence/death is just what it's most associated with/common.

To put it another (messy) way: If "snuff porn" is a facial shot, "guro" is bukkake.

Not every japanese thing has to be porn. Guro is not inherently sexual fetish. It's the same way as gun porn (usually) doesn't involve ducking someone with a gun and car porn... you get the idea. Attack on Titans and Tokyo Ghoul can be described as guro despite having almost no sexual content.
